To help us better understand your needs, please complete and return the appropriate intake form before your scheduled appointment. Intake forms may be emailed to info@eastpolklaw.com, delivered in person to our Altoona office, or securely uploaded using the form below.

You may also print and bring your completed form to your scheduled consultation.

Instructions:

• If your appointment relates to estate or trust matters, please complete the Probate Intake Form.
• For estate planning services, use the Estate Planning Intake Form.
• For guardianship consultations, download the appropriate Adult or Minor Guardianship Form.
• If you are meeting with us to discuss Real Estate, please complete the Real Estate Planning Form.
